Mr. Lam's Family: Early Preparation of Care Equipment through Medical-Social Collaboration

I am a family member of 85-year-old Mr. Lam. He previously suffered from bone cancer and fractures, resulting in weakness in the lower body. After returning home from the hospital, he needed long-term bed rest. Initially, the domestic helper and I were at a loss.

Although we already had a medical bed and a shower commode chair with wheels at home, we were deeply concerned about accidents during transfers and the physical strain on our helper. Fortunately, during Mr. Lam's hospital stay, the Occupational Therapist (OT) at Kowloon Hospital assessed the need for care equipment after discharge and suggested that we could rent equipment through the Jockey Club "age at home".

The services provided by the "age at home" team were comprehensive and seamless. The Care Equipment Counsellor measured the dimensions of the home environment and existing equipment, and directly contacted the Hospital’s OT to prepare the hoist and bed mattress before discharge. We rented an air mattress, a foldable hoist, and a bedside table, preventing pressure sores, facilitating daily transfers and bedside dining. The equipment significantly improved Lam's quality of life and safety, helping him adapt to home life more comfortably. The OTs from the "age at home" rental team trained my domestic helper to use the hoist, greatly reducing the caregiving pressure. We quickly adapted to Lam's daily nursing care with the help of “age at home”.

We are thankful to "age at home" for providing appropriate assistance. The "medical-social collaboration" for home assessment, equipment delivery, on-site assembly, instructional guidance, and follow-up were all well taken care of.

Mr. Choi’s Son: Adapting to Daily Life After Surgery

Hello everyone, I’m Mr. Choi’s son. My 90-year-old father recently underwent a lower limb amputation. His body is quite weak and struggle with self-care. He requires home care for daily life, meals and accompanying follow-up hospital appointments. As a new caregiver, I’m incredibly worried about not being able to take good care of him. We are grateful to have received assistance from “age at home” under the referral of the Pamela Youde Nethersole Eastern Hospital. The “age at home” rental team collaborated with the hospital’s Occupational Therapist to conduct a comprehensive assessment of my father and our home and recommend suitable Gerontech solutions.

We rented various assistive devices to prevent pressure injury, improve daily mobility and stay-at-home life, including smart hospital bed with turning position, pressure relief seat cushion, wheelchair with special function (recliner), and room shampoo & vacuum devices, making dad more comfortable. The team provided thoughtful hands-on guidance and follow-ups, helping us gradually learn and attune to the care for my father, allowing to quickly cohabitate after surgery with the pressure alleviated. We are very grateful for the "age-at-home" gerontech rental service, my father’s quality of life has improved significantly, and he’s even going out more often. It has truly given him a sense of hope.
